The Massachusetts Department of Public Health (DPH) Bureau of Community Health and Preventions Division of Community Health Planning & Engagement is issuing this Request for Response (RFR) to solicit proposals from eligible social service organizations (SSOs) for funding to support development and implementation of structural changes and ramp-up projects. The purpose of the Social Services Organization Flexible Services Preparation Fund is to support qualified social service organizations that are beginning to participate in the MassHealth Flexible Services Program with MassHealth Accountable Care Organizations (ACOs). Through this grant, DPH aims to prepare SSOs for successful partnerships with MassHealth ACOs by overcoming organizational and technological barriers that limit SSOs ability to provide ACO members with nutrition and/or housing support services that will improve the members health or prevent worsening of health outcomes and overall well-being and reduce the total cost of care for that member. Applicants can apply for funding to support structural changes and ramp-up infrastructure costs needed to partner with ACOs on the members health-related social needs (HRSN). Structural changes enable SSOs to implement system reforms that are sustainable beyond the life of the grant and include, but are not limited to, information technology or billing systems. Ramp-up costs are activities that prepare the SSO for managing ACO referrals and providing services such as updating of business practices and protocols, training for staff on quality improvement or new protocols, and legal and contracting supports.
